Take your bottle, and cut the bottom off, so that it is like a funnel. You can do this with a knife or scissors.

Take off the lid (but keep it safe for the bonus whistle at the end!).

I tried a couple of different sized balloons (water bombs included) and it seems to be the smaller the balloon the higher the pitch.

Take your balloon and cut them bottom off.

Then feed it in to the lid and turn it back on itself to seal the top.

Decorate

You can make your bottle look a bit prettier!

Use your duct tape in any way you want! I tried to make mine look like a football!

Make sure you wrap the duct tape around the cut part of the bottle so that there are no sharp edges!

Make Some Noise

Like I said, the different sized balloons make different noises.

This is the water bomb balloon:

This is the big balloon:

I think the water bomb is the better one!

Bonus Whistle

You can also use the lid of the bottle as a whistle.

You need to position your hands as per the photo, creating a v at the top. I have shown it with one hand only to help show the hand position - just mirror the other hand.

Then rest your lips on your knuckles and blow in to the v. It may take some practice but once you get it right you can make a really loud whistle!

Here is a demo:
